During our initial call, we’ll review your needs and preferences. Our RN case manager will then conduct an in-home assessment and will create a plan of care based on your physicians’ orders, with your input.
We can begin care as soon as the patient or their family has signed an agreement and consent for treatment, an initial assessment has been completed, and the patient’s physician has reviewed and signed off on our plan of care.
Adults who need in-home nursing care, whether it’s ongoing or for a limited time. We care for clients with a range of needs, including adults with developmental disabilities, those who are recovering from an injury or illness, and those who need daily nursing support.
Windward Home Health is private pay only, which means we bill you directly and do not accept insurance or Medicare. If you have long-term care insurance, that coverage may provide reimbursement.
All of our home health nursing care is provided according to your physician’s orders. Your physician must first examine any new conditions and then provide us with care orders.
Our visits are determined by your physician’s orders in consultation with you and/or your family. Most clients receive weekly or twice-weekly visits, but we can do more or less as required.
