Peter – Medically Complex Client
Peter, a 92-year-old man, lives in one of the apartment complexes he owns and has managed for many years with his staff. Peter never married or had children and has no living family in the area. A very kind and generous person, Peter has taken care of the people who work for him, and they remain involved in his life. His durable powers of attorney (DPOA) for both health and finances are former employees of his. However, these individuals have little to no healthcare experience and had difficulty navigating decisions for Peter’s care as he became older and had more complex medical needs.
Our Role
- The Aging Life Care Manager helped set up meal delivery, home monitoring systems, physical therapy sessions, and doctor visits, regularly communicating with Peter’s DPOAs as well as his Windward Home Care Aides to ensure Peter gets the best round-the-clock care.
- Home Care Aides assist with dressing, grooming, cooking, light housekeeping, shopping, medication reminders, etc., and provide companionship, reporting any concerns to his Aging Life Care Manager as appropriate.
- The RN Case Manager created a care plan and visits Peter every two weeks or more so Peter’s increasingly complex medical needs can be accommodated in the comfort of his own home.
- On an ongoing basis, the RN Case Manager conducts physical assessments, takes vital signs, refills pill boxes, and picks up prescriptions, coordinating Peter’s care needs with his Home Care Aides and doctors, making sure any changes his doctors prescribe are implemented.
