Janice – Facility Transitions for Progressive Alzheimer’s Care

Windward’s continuous care oversight throughout the progression of a client’s Alzheimer’s included coordinating multiple facility transitions, her transportation to appointments, and 24/7 care. Our constant presence to advocate for her needs proved invaluable for the fiduciary, ensuring their client always received safe, personalized, and compassionate support.
A Windward Aging Life Care Manager® began working with Janice at her La Jolla home in 2020, around the time of her Alzheimer’s diagnosis. Windward Home Health also provided medication management and assistance with doctors’ appointments. As Janice’s condition steadily progressed, requiring higher levels of care, our Aging Life Care Manager partnered with Janice’s fiduciary to ensure smooth transitions to assisted living facilities that could provide the support she needed.
Our Role
- Our Aging Life Care Manager helped the fiduciary move Janice into an assisted living community in spring 2021 in response to her increased 24/7 care needs and budget situation, while we continued to assist with transportation to all her medical appointments.
- Our care team remained involved in overseeing Janice’s day-to-day needs at the assisted living community until increased concerns over safety and falls necessitated full 24/7 care for her there.
- Once she became wheelchair bound, Windward set up wheelchair transport to all her medical appointments and attended her appointments to advocate for her.
- We worked with the fiduciary on another move two years later, this time to a smaller board and care facility where Janice could receive more individual attention. Our Aging Life Care Manager researched and toured several facilities until we found the best home for Janice, and then we coordinated all aspects of the move.
- Throughout the transitions, our Aging Life Care Manager and care team regularly visited Janice, assisting the fiduciary with any insurance-related needs as well as ordering all her clothing, shoes, make-up, and other personal items.
- Our caregivers, all highly trained Windward staff, continue to visit Janice three times a week to engage her in meaningful activities and to advocate on her behalf as her Alzheimer’s makes it more difficult for her to communicate her needs.
Outcome
Windward’s expertise and continuous involvement with Janice’s care – from Alzheimer’s care support through assisted living placement, board and care placement, and ultimately palliative and hospice care – provided indispensable support and peace of mind for Janice’s fiduciary through every stage of her care journey.
